Bold/Blue/Italicized

Fields that are Bold and blue in color are mandatory and must have data entered in order to save the record. The blue color indicates that these fields are also hyperlinks. The user may select the hyperlink to access a drop down selection window where they can select pre-defined data.

Blue/Italicized

Same description as above, except these fields are NOT mandatory.

Bold/Black

Fields that are Bold and black in color are mandatory. The user must enter/type data into the field in order to save the record.

Black

Information can be entered if desired. These fields are NOT mandatory.
