The Subject to be revised.

Employee NO

The Employee Number of the Manual Revision Requester.

Submitted By

The Employee submitting the Manual Revision Request.

Position

The Employee Position.

Contact Phone

A contact phone number for the Manual Revision Requester.

Reference checkbox

Check to enter additional reference information in the provided field.

Standard Practice

Standard practice notation.

Page

The page number associated with the revision.

Manual Chp

The Manual’s chapter number associated with the revision.

Par

The paragraph number associated with the revision.

Item

The item identifier associated with the revision.

Sub Par

The sub-paragraph associated with the revision.

Task Card

The Task Card / Job Card that details the scheduled maintenance work.

Figure

The figure identifier associated with the revision.

ATA

The ATA Chapter, Section, Paragraph related to the Manual.

Revision Correction Request

Enter the proposed revision information (i.e. the correction or modification to be made to the specified text).

Describe Benefit Of Proposed Revision

A description of the benefits associated with the proposed revision.
